Output Column Values requires a array values.
So please either use aggregator before using this list module, or utilize a text aggregator preceding the list module and apply the split function to convert it into an array…

I’ve made the updates; however when I ran the scenario it just kept running with no messages? Is this correct?
Update! The scenario ran successfully; however no form data was submitted into Monday.com

Please make a text with the column id with coma separation, you can use set variable in this case if it come from the same bundle.
Kindly create a text containing the column IDs separated by commas. If these IDs originate from the same bundle, you may use a set variable for this.
