Format date to month/year

Hey guys, I am looking into formatting the date to show ONLY month and year. What do I need to change in the current array to make this happen?

Could you add 2 screenshots:

• one for the output bundle of module 13 (Client folder)
• one for the data mapping of module 46 (Program Folder)?
