I want to insert the image and file link from Jotform into a Word template.
It doesn’t matter which one is easiest (Microsoft Word template or Google Doc).
I would appreciate your help.

Information from the form I created in Jotform
Data from Jotform
Raw Microsoft Word template in Google Drive
Create a fillable Microsoft Word template
The photo is not visible in my output file (docx and pdf), and the file does not appear as a url link.
I’m asking for help with this issue.
