Has anybody encountered this problem? Google docs Template

I am creating a doc from a template and using place holdeers. but when i create the doc and turn it into pdf it comes out with the first page as “Tab 1” (see attached), even though this is not in the original template.

1. How exactly do you create a PDF?
2. Are you sure everything is properly mapped and you are processing the exact doc that you created?
3. How does the doc look like before you create a PDF out of it?
4. Please, share more detail about every step and mapping between modules.

this is how i create the pdf

mapping if fine, doc looks like a normal doc with variables in it before pdf creation.(attcached)
