Facebook Leads Form data not loading under New Web hook

I wanted to create a new web-hook for a new scenario unfortunately the Facebook lead form is showing no data.

It is showing the connection > Fb Page, Next should be form data, but it is not able to receive the details.

Error code I am getting is (#200) Provide valid app ID (200, OAuthException)

I have checked fb settings for app integration, reintegrated the make app just to see if that works too. But no luck. Can anyone help?
